Pet travel health certificate questionnaire

Tell us about your trip so our team can prepare for your pet’s domestic or international travel consultation.

Before you start

Submit at least 48 hours before your veterinary appointment. Some destinations require weeks or months of preparation. Contact us early; submitting this form does not book an appointment or guarantee a certificate or travel approval.

You or your travel agent must confirm the destination and carrier requirements. If plans change after submission, send the updated details to the front desk in writing.

Have your itinerary, destination address and pet records handy. Use one questionnaire for pets sharing the same trip and contacts. For different itineraries, submit separate forms.

Required fields are marked with an asterisk (*). Leave unknown record dates blank and explain in the notes.
